Human error occurrences in maintenance affect reliability, availability and safety of repairable systems.
The paper performs analysis of human error servicing (HES) considering its impact on maintenance. HES is identified as one of the major causes of imperfection in maintenance, as well as potential risk generator.
Human error servicing mode effect analysis (HESMEA) is used for HES risk quantification in a maintenance organization. The paper extends the weighted FMEA approach with comparative weights for different HES modes. The work proposes five RPN approaches for multiple HES modes and provides a modified 3-D HESMEA matrix for HES.
The proposed approaches reduce the subjectivity and uncertainty of traditional FMEA. HES modes are ranked jointly on the basis of RPN and expert judgments.
Overall the paper suggests ways for maintenance performance improvement through human error servicing reduction.
The work is applicable to all maintenance systems, organizations with human interventions.
