This study aimed to demonstrate the epidemiology and spatial characteristics in rate of different serum levels of anti-hepatitis B surface antibodies (HBsAb) in college students in a University in Hunan Province, China. A total of 4648 college students with complete data of serum levels of anti-hepatitis B surface antibodies and geographic information were included in the present study. Subjects were categorized into three groups according to the serum level of HBsAb: negative, weak positive, and positive. Totally, 46.9% of the students were HBsAb negative, 36.4% weak positive (35.9% and 36.6% for males and females, respectively), 16.7% positive (17.7% and 16.3% for males and females, respectively). There is a significant difference in the geographic distribution of the serum level of HBsAb of the study population. Also, we found a sex difference in the clustering of low-low area of rate of HBsAb (+) in Hunan Province. It is likely that we should develop area-specific and sex-specific intervention and prevention strategies for different areas of the Hepatitis B infection in Hunan Province, China.
